The Newbie Guide To Understanding Your Health Insurance Policy Part 2


Welcome to part two of our series of articles titled "The Newbie Guide to Understanding Your Health Insurance Policy". If you read out previous article then you know that we've already discussed some of the most important terms when it comes to deciphering your health insurance plan. Our first article covered the deductible, co-payments (sometimes referred to as co-insurance) and out-of-pocket costs. This article will cover exclusions, pre-existing conditions as well as the waiting and grace periods. Let's get started.

As with any business, your health insurance company or provider does have a requirement of needing to make a profit. With that said, many health insurance plans come with a lifetime maximum cap or term placed on it by the provider of your health insurance policy. What this generally means is that at some point in time (based on a dollar amount) your health insurance policy will cease to provide coverage. Initially this can sound very alarming but actually you don't really need to be worried. This cap normally has a very high ceiling or figure amount that is in place so that your health insurance can provide adequate coverage for your medical needs. Even with the high cost of medical care in today's economy you can still place a safe bet that a majority of consumers with health insurance benefits will never exceed their cap that has been place don their policy by their health insurance provider.

The exclusions section is an important section of your health insurance plan and should be read rather carefully in order to familiarize your self with what your health insurance policy will and will not cover. Don't let this be a gray area for you. Take the time to do your due diligence in order to make sure you know exactly what your health insurance policy provides to you in terms of health coverage. The last thing you want to find out is that the fees associated with the recuperation period needed in a hospital after a major operation or surgery isn't covered by your health insurance. If you were told that this section was the most important section of your whole entire health insurance policy it wouldn't be to far from the truth. Don't make a mistake and just gloss over or skim this section. Take the necessary time to really learn what it covers because it could save you a lot of money in the future.

If there ever were a nemesis for folks wanting to obtain a new health insurance policy then pre-existing conditions would be it. Basically this portion of your health insurance policy deals with any previously existing medical conditions that you may have. In some cases many health insurance companies will go ahead and assume the risk associated with covering a pre-existing condition, of course this naturally means that you could be faced with paying a higher deductible and having a lower amount of coverage. However, don't be mislead - there are some instances where a pre-existing condition could result in a health insurance provider refusing or denying you the opportunity to receive medical coverage under their health plans. Make sure and do your homework when you shop around to see what pre-existing medical conditions are covered and by which health insurance provider.

A few minor health insurance terms include the waiting period and the grace period. The waiting period is the length of time necessary before the benefits from your health insurance policy actually kick in and start providing coverage. The wait times differ among the many health insurance companies so don't forget to compare the waiting period when doing your health insurance comparison-shopping. The grace period deals with finances and it's the amount of time allowed for a consumer to render payment for their health insurance rate or premium after the original payment deadline has passed.

As you can see there are many different aspects to a health insurance policy that should be checked, reviewed and compared by someone interested in obtaining a good solid health insurance coverage plan for the right amount of money.

The Stop Milking Cancer Campaign ends today, 24 September. Thanks to you, our readers and anti-cancer advocates,  Breast Cancer Action (BCA) attained and even surpassed their targets for this campaign.


If you recall the post a couple of weeks back, Stop Milking Cancer Campaign was launched on September 1 to call on rBGH maker Eli Lilly to stop manufacturing this cancer-causing hormone and break the “cancer profit cycle.”


Here is what BCA has to report about the campaign:

We’re joined by 10 campaign partners, including Corporate Accountability International, Our Bodies Ourselves, Healthy Child Healthy World, and AllergyKids Foundation.We’ve reached our goal of gathering 6,220 signatures on the petition we’ll be sending to Eli Lilly’s leadership – that’s one for every day rBGH has been on the market.BCA plans to plaster our message to Eli Lilly in a very public way – on a billboard in Indianapolis, where Eli Lilly’s headquarters are located.We’ve had a robust presence on blogs, Facebook, and Twitter!

However, just because the target of 6,220 signatures (representing the number of days rBGH has been on the market) has been achieved, that doesn’t mean that we should stop here. How to Become a Home Health Care Nurse


Home Health Care Nursing Information and Overview

Home health care is allowing the patient and their family to maintain dignity and independence. According to the National Association for Home Care, there are more than 7 million individuals in the United States in need of home health care nurse services because of acute illness, long term health problems, permanent disability or terminal illness.

Home Health Care Basics

Nurses practice in a number of venues: Hospital settings, nursing homes, assisted living centers, and home health care. Home health care nursing is a growing phenomenon as more patients and their families desire to receive care in their homes. The history of home health care stems from Public Health Nursing where public health nurses made home visits to promote health education and provide treatment as part of community outreach programs. Today academic programs train nurses in home care and agencies place home health care nurses with ailing individuals and their families depending on the nurse's experience and qualifications. In many cases there is a shared relationship between the agency and the academic institution.

Many changes have taken place in the area of home health care. These include Medicare and Medicaid, and Long Term Care insurance reimbursement and documentation. It is important for the nurse and nursing agency to be aware of the many factors involved for these rules and regulations resulting from these organizations. Population and demographic changes are taking place as well. Baby boomers approaching retirement and will present new challenges for the home health care industry. Technology and medical care in hospitals has lead to shorter inpatient stay and more at-home rehabilitation. Increases in medical outpatient procedures are also taking place with follow-up home care. This has resulted in the decrease of mortality rate from these technologies and medical care has lead to increases in morbidity and chronic illness that makes the need for home health care nursing a greater priority.

Home Health Care Nurse Job Description

Through an array of skills and experience, home health care nurses specialize in a wide range of treatments; emotional support, education of patients who are recovering from illnesses and injury for young children and adults, to women who have experienced recent childbirth, to the elderly who need palliative care for chronic illness.

A practicing nurse must have the skills to provide care in a unique setting such as someone's home. The nurse is working with the patient and the family and must understand the communication skills for such dynamics. Rapport is evident in all nursing positions, but working in a patient's own living space needs a different level of skill and understanding. There is autonomous decision making as the nurse is no longer working as a team with other nurses in a structured environment, but is now as a member of the "family" team. The host family has cultural values that are important and are different for every patient and must be treated with extreme sensitivity. Other skills include critical thinking, coordination, assessment, communication, and documentation.

Home health care nurses also specialize in the care of children with disabilities that requires additional skills such as patience and understanding of the needs of the family. Children are living with disabilities today that would have resulted in mortality just twenty years ago. Genetic disorders, congenital physical impairments, and injury are just a few. Many families are familiar with managing the needs of the child, but still need expert care that only a home health care nurse can provide. It is important that a home health care nurse is aware of the expertise of the family about the child's condition for proper care of the child. There are many complexities involved, but most important, a positive attitude and positive reinforcement is of utmost importance for the development of the child.

Medication coordination between the home health care nurse, doctor, and pharmacist, ensures proper management of the exact science behind giving the patient the correct dose, time of administration, and combinations. Home health care nurses should be familiar with pharmacology and taught in training about different medications used by patients in the clinical setting.

Many advanced practicing nurses are familiar with medication regiments. They have completed graduate level programs. Home health care agencies believe that a nurse should have at least one year of clinical experience before entering home health care. Advanced practicing nurses can expedite that training by helping new nurses understand the home health care market and teaching.

Employment and Salary

According to the United States Department of Labor, there were 2.4 million nurses in America, the largest healthcare occupation, yet many academic and hospital organizations believe there is a gross shortage in nursing staff. The shortage of nurses was 6% in 2000 and is expected to be 10% in 2010. The average salary for hospital nursing is $53,450 with 3 out of 5 nursing jobs are in the hospital. For home health care, the salary is $49,000. For nursing care facilities, they were the lowest at $48,200.

Training and continuing education

Most home health care nurses gain their education through accredited nursing schools throughout the country with an associate degree in nursing (ADN), a Bachelor of Science degree in nursing (BSN), or a master's degree in nursing (MSN). According to the United States Department of Labor, in 2004 there were 674 BSN nursing programs, 846 ADN programs. Also, in 2004, there were 417 master's degree programs, 93 doctoral programs, and 46 joint BSN-doctoral programs. The associate degree program takes 2 to 3 years to complete, while bachelors degrees take 4 years to complete. Nurses can also earn specialized professional certificates online in Geriatric Care or Life Care Planning.

In addition, for those nurses who choose to pursue advancement into administrative positions or research, consulting, and teaching, a bachelor's degree is often essential. A bachelor's degree is also important for becoming a clinical nurse specialist, nurse anesthetists, nurse midwives, and nurse practitioners (U.S. Department of Labor, 2004).

All home health care nurses have supervised clinical experience during their training, but as stated earlier advanced practicing nurses hold master's degrees and unlike bachelor and associate degrees, they have a minimum of two years of post clinical experience. Course work includes anatomy, physiology, chemistry, microbiology, nutrition, psychology, and behavioral sciences and liberal arts. Many of these programs have training in nursing homes, public health departments, home health agencies, and ambulatory clinics. (U.S. Dep. of Labor, 2004).

Whether a nurse is training in a hospital, nursing facility, or home care, continuing education is necessary. Health care is changing rapidly and staying abreast with the latest developments enhances patient care and health procedures. Universities, continuing education programs, and internet sites, all offer continuing education. One such organization that provides continuing education is the American Nurses Association (ANA) or through the American Nurses Credentialing Center (ANCC).

Conclusion

There are many rewards to becoming a home health care nurse. Some rewards include the relationship with a patient and their family, autonomy, independence, and engaging in critical thinking. The 21st Century brings with it many opportunities and challenges. We must meet these challenges head on - there is an aging baby boomer population, a growing morbidity factor due to increased medical technology and patient care, and the growing shortage in nursing care.

Becoming a home health care nurse today is exciting and an opportunity to make a difference one life at a time. With clinical experience and proper education, a home health care nurse will lead the future of medical care.

The Increasing Surge of Health Care


While sitting back in her blue jeans and wearing a heavy workout sweater at the Legacy Emanuel Hospital's Emergency room, Angela Jones has her feet prompted up and crossed atop of a small table. When asked about health care issues and how they affect her, Angela explains that there is a portion of people who suffer from not having health care insurance. She makes it clear that some of those who suffer most are young people. Jones, who is a college student, declared her passion for the young because it falls under her own age group.

Says Jones, "The Oregon Health Plan should be open to more people who are under 21 years old. Private insurance shouldn't be so expensive for young people."

According to national surveys, the primary reason people are uninsured is the high cost of health insurance coverage. Notwithstanding, nearly one-quarter (23 percent) of the uninsured reported changing their way of life significantly in order to pay medical bills. Economists have discovered that increasing health care costs correlate to drops in health insurance coverage.

Jones believes that some of the greatest challenges that people face across this nation is obtaining affordable health care. "I would open an Oregon Health Plan to a variety of people who don't have insurance. It is hard to get health insurance."

Terri Heer, a registered nurse at a local hospital, claims that in order to improve America's health care system a key ingredient is to "make sure that everyone (has) access."

This would include cutting out on expenses that are not palpable to so called "health care needs". Heer says, "First, we spend a lot of money servicing people for illnesses that can be prevented. Some of the money spent can go to other things."

Over the long haul, should the nations health care system undergo significant changes, the typical patient may not necessarily see the improvements firsthand. "I would love to say there will be a lot of changes. I am not a pessimist, but I don't think there will be any change," says Heer. Heer does allude to the fact that if more money were spent for people in the health care arena, she says that there is a possibility that the necessary changes would be more evident.

Whether health care is affordable or not is an issue that affects everyone. According to a recent study last year, health care spending in the United States reached $2.3 trillion, and is projected to reach $3 trillion by 2011. By 2016, it is projected to reach $4.2 trillion. Although it is estimated that nearly 47 million Americans are uninsured, the U.S. spends more on health care than any other nation.

The rising tide of health care stems from several factors that has an affect on us all. First, there is an intensity of services in the U.S. health care system that has undergone a dramatic change when you consider that people are living longer coupled with greater chronic illnesses.

Secondly, prescription drugs and technology have gone through significant changes. The fact that major drugs and technological advancement has been a contributing factor for the increase in health care spending. Some analysts suggest that the improvement of state-of-art technologies and drugs increase health care spending. This increase not only attributes to the high-tech inventions, but also because consumer demand for these products has gone through the roof, so to speak.

Thirdly, there is an aging of the population. Since the baby boomers have reached their middle years, there is a tremendous need to take care of them. This trend will continue as baby boomers will qualify for more Medicare in 2011.

Lastly, there is the factor of administrative costs. Some would argue that the private sector plays a critical role in the rise of health care costs and the economic increase they produce in overhead costs. At the same time, 7 percent of health care expenses are a result of administrative costs. This would include aspects of billing and marketing.

Terra Lincoln is a woman who was found waiting in the Emergency room at the Providence Portland Medical Center. When asked about the rising costs of health care, she said, "If you don't have medical coverage, it'll cost you too much money. If I leave the hospital right now and I need to buy two (types) of medicines, I couldn't afford it." Lincoln says that she is a member of the OHP, but she believes that there are still issues that need to be addressed.

Terra recognizes that to reduce medical costs, she would have to start by getting regular checkups. "Sometimes people of color wait till they're in pain before they get a checkup," she said.

A national survey shows that the primary reason why people cannot afford health care is because of soaring costs of health care coverage. In a recent Wall-Street Journal-NBC survey it is reported that 50% of the American public claims that their highest and most significant economic concern is health care. Consequently, the rising cost of health care is the number one concern for Democratic voters.

Regarding the rising tide of health care, Kristin Venderbush, a native Wisconsin, and another patient in emergency at Providence says, "I worry a lot about what happens to the working poor. They don't have OHP. If you can't advocate for yourself, you will not get the health care you need...on every level."

Harvard University researchers conducted a recent study that discovered that the out-of-pocket medical debt for an average consumer who filed bankruptcy was $12,000. This study noted that 68 percent of those who had filed for bankruptcy carried health insurance. Apparently, these bankruptcy's were results from medical expenses. It was also noted in this study that every 30 seconds someone files for bankruptcy after they have had some type of serious health problem.

In spite of all the social and economic bureaucracy in the health care arena, some changes were made in Washington on January 28, 2008. In his State of the Union address, President Bush made inquired Congress to eliminate the unfair bias of the tax code against people who do not get their health care from their employer. Millions would then have more options that were not previously available and health care would be more accessible for people who could not afford it.

Consequently, the President believes that the Federal government can make health care more affordable and available for those who need it most. Some sources suggest that the President not only wants health care to be available for people, but also for patients and their private physicians so that they will be free to make choices as well. One of the main purposes for the health care agenda is to insure that consumers will not only have the freedom to make choices, but to also enable those to make decisions that will best meet their health care needs.

Kerry Weems, Acting Administrator of the Centers for Medicare and Medicaid Services, oversees the State Children's Health Insurance Program, also known as SCHIP. This is a critical program because it pays for the health care of more than six and a half million children who come from homes that cannot afford adequate health insurance. These homes exceed the pay scale for Medicaid programs, therefore are not able to participate.

During SCHIP's ten year span, states have used the program to assist families with low-income and uninsured children for their sense of well-being in the health care arena. The Bush Administration believes that states should do more of an effort to provide for the neediest children and enable them to get insurance immediately. The SCHIP was originally intended to cover children who had family incomes ranging from $20,650. This amount would typically include a family of four. According to sources, all states throughout the U.S. have SCHIP programs in place and just over six million children are served.
